The Caddo Nation’s Community Health Program is funded by Indian Health Services on a yearly basis to assist Caddo tribal members with their health needs. The CHR Program’s purpose is to assist eligible Indian individuals and families within our service are to gain access to comprehensive health care services.
All CHR staff members are trained in CPR, First Aid and EMS First Responder.
The CHR Program is often a vital link to health care services for our Indian people in rural areas. Our work includes making and rescheduling appointments for clients, pick up and delivery of medications, home visits, case screening and management, monitoring of patients, and health education.
The staff transports clients who are without transportation to doctor’s appointments at the Anadarko IHS clinic or the Lawton IHS hospital, El Reno or Clinton. We can also provide transportation to outside referrals made by a doctor from IHS. The CHR Office needs to know 2 to 3 days in advance about appointments, refills of prescriptions, and transportation needs. There is a call-in procedure for requesting medication at the Lawton Indian Hospital; call toll-free 1-888-275-4886 for assistance, and allow 2 days before picking up medicines.
The I.H.S. clinics and Lawton Hospital are computerized and if you have refills on medications, they can now be filled at the Anadarko Pharmacy. You may call them in and let us know if you need a CHR to pick them up for you, or let us know and we can request them for you. Also the Anadarko & Carnegie clinics and Lawton Indian Hospital are doing same day appointments. You must call in each morning starting at 7:45 a.m. to get an appointment for that day. The CHR Director also handles Tribal Health Services, which provides assistance up to $100 for eyeglasses, dentures, health equipment, cost of prescriptions, and hearing aids. Assistance can be provided to individuals once per year.
The Caddo Nation’s Diabetes Program is also administered through the CHR Office. Funds are available to assist tribal members who have diabetes with eyeglasses, dentures, hearing aids, and medical supplies. Assistance can be provided to individuals once per year. See below for an application.
TRIBAL ASSISTANCE IN HEALTH SERVICES
The Caddo Tribal Council has appropriated funds from the motor fuel monies to assist tribal members in health services for the fiscal year beginning each October 1, and ending September 30. Assistance is a one-time only, per Fiscal year for eligible enrolled members, the limit will be $100. Dental, prescription and medical bills must be current, for a period of time, after October 1.
The categories which is approved for assistance is:
Eyeglasses may be obtained at any optometrist and any amount over $100.
